ESI ESIC Benefits India — What Employees Get from ESIC 2026
The Employees' State Insurance Corporation (ESIC) is India's largest social insurance organisation — covering over 14 crore insured persons. ESI is mandatory for employees earning up to Rs. 21,000 per month in covered establishments. Despite being one of the most valuable employee benefits, many HR teams do not explain ESI benefits clearly to employees — resulting in low utilisation and employee dissatisfaction.
This guide explains every ESI benefit so that HR teams can communicate it effectively and employees can use it fully.

Who Is Covered Under ESI?
- Employees of establishments with 10 or more employees (20 in some states)
- Monthly gross wages up to Rs. 21,000 (Rs. 25,000 for persons with disability)
- Coverage continues for 26 weeks after a salary increase takes gross above Rs. 21,000 (contribution period protection)
- Seasonal factories: coverage if employed for at least 78 days in a year

- Employee: 0.75% of gross wages
- Employer: 3.25% of gross wages
- For an employee earning Rs. 15,000/month: employee pays Rs. 113, employer pays Rs. 488 — total Rs. 601/month to ESIC

ESI Benefits — Complete List
1. Medical Treatment (Most Used Benefit)
- Free outpatient treatment at ESI dispensary near home or workplace
- Free hospitalisation at ESI hospital or empanelled private hospitals
- Specialist treatment: referral from dispensary to ESI hospital specialist
- Diagnostic tests: free at ESI facilities
- Medicines: free at ESI dispensary pharmacy
- Coverage: insured person AND their family members (spouse, children, dependent parents)
- No waiting period after 9 months (Wage Period 1)
2. Cash Sickness Benefit
- When: employee is unwell and cannot work
- Amount: 70% of daily wages for up to 91 days in two consecutive contribution periods
- Certificate: from ESI medical officer
- Application: Form 13 to ESI branch office or online through ESI portal
- Example: employee earning Rs. 15,000/month gets Rs. 350/day sickness benefit during illness
3. Extended Sickness Benefit
- For specified long-term diseases: tuberculosis, mental disease, cancer, kidney disease, epilepsy, etc.
- Amount: 80% of daily wages
- Duration: up to 2 years (730 days)
- Requires: continuous coverage for 2 years before claim
4. Enhanced Sickness Benefit
- For employees undergoing vasectomy or tubectomy (family planning operations)
- Amount: 100% of daily wages for 7 days
- No contribution period requirement
5. Maternity Benefit
- Duration: 26 weeks (12 weeks for 3rd child and beyond)
- Amount: 100% of daily wages during maternity leave
- Application: Form 19 through ESI dispensary
- Note: For ESI-covered employees, the employer is NOT separately liable to pay maternity benefit — ESIC pays directly
- Medical bonus: Rs. 5,000 if delivery not taken in ESI hospital
6. Disablement Benefit
- Temporary Disablement: if workplace accident causes temporary inability to work
- Amount: 90% of daily wages
- Duration: entire period of disablement
- Permanent Disablement: if accident causes permanent incapacity
- Amount: 90% of daily wages × % of disablement (assessed by medical board)
- Paid as a pension for life
7. Dependants' Benefit
- If insured person dies due to employment injury: pension to dependants
- Widow: 3/5 of full benefit rate, for life (or until remarriage)
- Children: 2/5 of benefit rate, until 25 years of age
- Parents: if wholly dependent on deceased, share of remaining benefit
8. Funeral Expenses
- Rs. 15,000 paid to person performing last rites of insured person
- Claim within 3 months of death
9. Vocational Rehabilitation
- For permanently disabled insured persons
- Free vocational training to develop new skills for gainful employment
10. Confinement Expenses
- If delivery is in an area not serviced by ESI hospital: Rs. 7,500 reimbursed

How to Access ESI Benefits
Step 1: ESI Card
Employer registers employee on ESIC portal → ESIC generates IP number → Employee gets digital ESI card (downloadable on Umang app)
Step 2: Dispensary Registration
Employee registers at nearest ESIC dispensary with ESI card
Step 3: Use
- OPD: visit registered dispensary with ESI card
- Hospitalisation: dispensary gives referral to ESI hospital
- Cash benefits: apply online at esic.gov.in or at branch office
